Two of the most common questions I hear in my Plano office: “Should I get LASIK or SMILE eye surgery at Visionary Eye?” and “What is the difference?”
Both are excellent procedures. Both can give you the freedom from glasses and contacts you are looking for. But they are not identical, and the right choice depends on your eyes, your lifestyle, and your specific anatomy.
How LASIK Works
LASIK uses two lasers. The first, a femtosecond laser, creates a thin flap on the surface of the cornea thickness matters. That flap is lifted, and the second laser, an excimer laser, reshapes the corneal tissue underneath to correct your vision. The flap is then laid back in place and heals naturally.
LASIK has been performed for decades. It has an extraordinary safety record, treats a wide range of prescriptions including nearsightedness, farsightedness, and astigmatism, and LASIK recovery timeline is typically fast. Most patients see clearly within hours.
How SMILE Works
SMILE takes a different approach. A single femtosecond laser creates a small disc-shaped piece of tissue, called a lenticule, inside the cornea. That lenticule is then removed through a tiny incision, roughly 2 to 4 millimeters. There is no flap.
Because SMILE does not create a flap, it preserves more of the corneal surface structure. This can mean less dry eye after surgery and a cornea that retains more of its biomechanical strength.
The Key Differences
LASIK treats nearsightedness, farsightedness, and astigmatism. SMILE currently treats nearsightedness and astigmatism. If you are farsighted, LASIK is typically the better choice.
LASIK offers wavefront-guided and topography-guided treatment profiles that can address very specific corneal irregularities. SMILE is a newer procedure with a growing body of excellent outcomes data.
Dry eye after surgery tends to be less common with SMILE because the smaller incision disrupts fewer corneal nerves.
Recovery speed is similar for both. Most patients are back to normal activities within a day or two.

Sometimes LASIK is the clear winner. Sometimes SMILE is the better fit. Sometimes neither is ideal, and we look at ICL or PRK instead. The point is that the recommendation comes from the data, not from a business preference.
